Welcome and thank you for your interest in this study! This study is part of the Strengthening a Palliative Approach to Long-Term Care project (SPA-LTC) taking place across Canada.
Our Goals: Researchers from the University of New Brunswick and Université de Moncton would like to learn about your experiences with care during the last month of life in NB long-term care (nursing homes). Sharing your experience will help us to understand what is being done very well, and what could be improved.
This survey is for you if you are:
- A family member or friend,
- Of a person who lived in a long-term care nursing home in New Brunswick and,
- Died or passed away (anywhere e.g., hospital, nursing home)
- After January 1, 2019
This survey is completely voluntary. It will take approximately 30 minutes. You can stop the survey and save it at any time. This survey is anonymous. No individual answers will be shared with the nursing homes. If you would prefer to complete this survey on paper or over the telephone please call 1-833-777-6135.
This survey includes questions about your opinions of the care your family member or friend received from the nurses, staff, volunteers, and doctors in a nursing home in the last month of their life before they died. There are also demographic questions. This questionnaire contains a list of questions considered important in quality end-of-life care.
We understand that can be very difficult to answer questions about someone you loved or cared for that has died. We appreciate your consideration and time to complete this questionnaire. This questionnaire may bring up feelings like sadness, grief or anger. Please take your time and stop for a break if needed. Here is a list of free resources for support that you can contact.

Your answers are confidential and will not be shown to nurses, doctors or staff at the long-term care (nursing) home. We (the researchers) will group all the surveys we receive together so you cannot be identified. There are no right or wrong answers. Completely honest answers are most helpful!
