UNB upholds its commitment to privacy and the protection of personal information. It is important you understand why we are collecting your personal information and how it will be used.
Completion of the COVID-19 self-isolation form is mandatory for any students travelling to New Brunswick from outside of Canada or the Atlantic provinces (New Brunswick, Nova Scotia, Prince Edward Island, and Newfoundland). UNB is required by the Government of New Brunswick to check-in with students daily upon arrival to New Brunswick from outside of Atlantic Canada, and to ensure a self-isolation plan is in place for your own safety and well-being and for the safety of others.
The personal information collected in the COVID-19 self-isolation form will be used to contact you, to help UNB prepare for your arrival, and for internal planning and operational purposes related to COVID-19. It may also be used to coordinate a COVID-19 test, if required.
Access to the information you provide to us will be restricted to a limited number of employees involved in the coordination of self-isolation planning and contacting you for the daily check-ins. It may also be available to employees involved in operational planning related to and stemming from COVID-19.
There may be instances where the information collected on this form may be shared with the Government of New Brunswick Public Health for testing, and to assist with planning and prevention measures related to the COVID-19 pandemic.
The information collected on the COVID-19 self-isolation form is collected under the authority provided for in the New Brunswick Right to Information and Protection of Privacy Act.
