Survey for People Who Work with Children with Limb Loss

Estimated completion time: 10-15 minutes

This survey is intended for anyone who works with children missing a part of the upper limb(s). We hope to compile information from various regions and countries about how children with upper limb loss are treated for their functional needs.  We would like to share this information on the Handsmart website (www.handsmartgroup.org) so that clinicians or other interested persons can determine what to expect for rehabilitation services in the area in which they live as well as other areas of the world.

This survey is anonymous.

The record of your survey responses does not contain any identifying information about you, unless a specific survey question explicitly asked for it.

If you used an identifying access code to access this survey, please rest assured that this code will not be stored together with your responses. It is managed in a separate database and will only be updated to indicate whether you did (or did not) complete this survey. There is no way of matching identification access codes with survey responses.
